Engine Help

I have a 2001 Intrepid SE 2.7 Liter and I have noticed the water is in the oil. Is this usually a blown head gasket or is there anything else it could be ? Where can I find a diagram to help me along to change the head gaskets out ? I can work on older cars but these newer ones are smarter than I am. Can someone Please Help Me. Thank You
