Default 2nd Torque Converter, what's REALLY wrong?

Hey,
Hope everyone had a Merry Christmas! My Magnum spent Christmas at the Dealer... This is the 3rd time I've had my Magnum in the shop this fall for Transmission related issues. This is the 2nd time I went through a Torque Converter in less than 250 miles. I only have about 1,000 miles left on my Warranty and would like to be sure they fix it right.

1) 1st felt like the Rear End had a knock in it (like something was in the back and hit the rear of the car on brisk take off from stop)
2) Felt a vibration (near the console or the back of my seat) in the Transmission at High Speeds (lower than the speed limiter)
3) Hit traffic one night and idled homefor 20 miles in about 2 1/2 hours...
4) Felt Transmission begin to slip the next day. Put in gear and the Car wouldn't move. Engine would rev, but would not go...
5) Took it in, was told an Adapter Plug came out... Put in new adapter plug, filled with Fluid, was given back to me.
6) A week later, same thing. Thud in the back, vibration at High Speeds. Began to slip the next day, transmission began to smoke on my way home...
7) Took it in, was told they replaced the Torque Converter, a bunch of Clutches in the Transmission and a bushing at the Rear End.
8) Felt the thud in the back and a slight vibration / slip while driving away from the dealer...
9) Within 100 miles all of the symptoms were back. This time I could feel a Vibration while sitting still, at about 750 RPM.
10) It is now back in the shop...

What do you guys think is wrong? Has anyone else had issues like this?

maybe a bad pilot bushing in the end of the crank?this sounds like alignment issuses between the block and trans bell housing or between the torque converter and the crank.
