EGR Issues-2005 Magnum hemi

2005 Dodge Magnum 8 cyl Hemi; EGR has been replaced in shop 4 times in past year. Check engine lite on again, failed emission test, There are 2 codes currently: P0404 and P1521. Thoughts???
