Default Trans Shifter Problem!!!

O.K. here’s the deal, 2003 2500 CTD. Automatic Trans 48RE. 90+K miles.
From day one it seemed every now and then when I would shift the truck from Park to Rev. or Drive, it would get messed up and the detent or were the shift lever should be in drive it would actually display and be engaged in Neutral. So in order to get it to go into Drive I would have to put the shift lever in the second gear (2) location. Now everything seemed fine, but first it’s obviously not right, and the lever is practically on my knee, if I bump the lever I can accidently pop it into Neutral (N), it’s just not right. Anyway as of about two months ago it’s gone from every now and then to all the time. I read about adjusting the shift cable and brake sensor thing, to no avail. It seems that it will work as it should once then it’s back to being screwed up. Is the cable/rod, stretched or damaged? Do I need to adjust the shift deal inside the trans cover. I will be changing the fluid again in about two-three weeks so if needed I can take a look inside (pan off). I’ve adjusted the thing about 30 times and every time it either doesn’t work at all or works once then it’s back the way it was. This issue is driving me crazy, please help.
