Default Transmission slips at take off

I have a 01 24V Turbo diesel it has 6 speed manual transmission with 141k miles. At a take off it will slip just the slightest bit then grab and go. Only does this from a dead stop and doesn't matter if I am towing or not. Anyone have a similar situation happen to them? Could this be the clutch getting ready to bite the dust?

Forgot to mention it is a 4X4.
