Leaf spring shackles rotted and snapped
I am not sure what you are wanting specificaly.
To find the part you may try some local salvage yards, the local auto part stores, or last resort your local Dodge Dealership.
For replaceing the shackel I would jack up the frame before & aft of the spring with the broken shackel, remove the broken pieces and install the replacement.
To fix the buldge in the floor I would have a friend help you hammer it back down. One will need to be under the van with the backing bar as the other hammers the buldge out. You will be able to get most of it back out, but do not be suprised that it all may not hammer out flat again.
Do check the rest of the shackels to be sure that they are in good shape.

You can get new shackle receiver triangle brackets at the dealership. If you have rust in the rear shackle areas you may find yourself with a very difficult task of removing the three bolts that hold the receiver brackets to the body. If the rust is really bad your van may have approached the end of it's service life. Unless you're a good fabricator/welder repairs in this area can easily exceed the value of the van.
