Bad brakes on my 94 ram van
My van would pull to the right when the brakes were applied, changed the pads, bleed the lines, got new rear adjusters and things were better, but still a slight pull and the pedal wasn't as good like the year before.
Answer ... One of the brake lines aluminum tubing got pinched by the front suspension. This put everything out of whack for braking. Once fixed, the pressure and the pulling went away.

Thanks for the responses. I have tried most all of the suggestions above. I do have a mityvac which I have tried and I did start bleeding with the RR wheel (several times). The fluid is new and clear and is dot3. The brakes do not pull one way or the other, they simply do not stop well. The rears are also adjusted properly and the emergency brake is working properly too.
I will check into the RWAL accumulator valve since that is the only thing I did not look into.
I'll post an update as soon as I know anything new.

I greatly improved my brakes by getting more aggressive front pads.
Bendix titanium metallic 2.
BUt they are noisy from 1 mph top dead stop. Not screeching, but low pitched vibration that is hard to avoid.
I'll take the noise over having brakes that can barely stop the vehicle.
