Default Thermostat Replacement
How difficult is it to change the thermostat on a 1997 Dodge B3500 Van?

It's not difficult but it's a pain in the butt. Plan on moving both the alternator and the A/C bracket out of the way.

Is it easier to change from the outside of the van or do you have to get it from inside?

It's much easier from the outside. From the inside the throttle body is in your way and there's a lot less working room.
