I have a 93 B250 conversion with 230,000 it shifts hard in all gears and up until yesterday it would shift into I believe would be it's final range at 38 mph now it's shifting at 40 to 45 mph. Am I looking at a trans band adjustment or throttle rod, lever adjustment or worse? Any help would be greatly appreciated.

First do a fluid/filter change and adjust the bands. And also check the throttle valve cable/rod.
