1996 Dodge Van Smog help needed
Gross polluter, now you get to do a smog check every year instead of every 2 years.
You catcon might be toast. You are definitely running rich, and the high NOx can be due to too high of combustion temperatures.
Run a can of CRC guaranteed to pass through a 15 gallons of fuel, try a water mist decarbonization, check the fuel pressure, change the oil before the next test.
Techron and Redline sl-1 and gumout Regane are good fuel system cleaners too containing good amounts of PEA. Look for vaccuum leaks and test the resistance on the 2 wire temp sensor

You may have a test port on the fuel rail or you will need a special kit (or have a Dodge dealer) to connect to the fuel supply line and check the pressure.
time to revisit this... looking at the suggestions here, I am seeing a catalytic convertor and fuel pump are my most likely issues?

In all reality though, it's a bit hard to tell you what wrong simply by reading numbers on a page with no other symptoms listed.
How long since the last tune-up?
There's over 200K on the odometer, has the engine ever been rebuilt?
A valve job ever done?
Is the catalytic converter original?
Are the O2 sensors original?
Any trouble codes in the computer?
Etc, etc, etc...

ok.... so after doing tons of work... I am getting a p0135
which usually points to the first o2 sensor.... but that one is new and I no longer have the old one..... both o2 sensors are new, as is the cat

is this something I can test with my code reader?
from my understanding, this code can also be triggered by the front and rear sensor saying that they both read the same reading, and that is not allowed
