Default New Codes

All,
My van has been running a little rough, so I had it coded this AM.

It runs great, but when it's at operating temp, and at idle, it runs really rough, as if it is getting starved for gas, and eventually stalls. If I catch it in time before it stalls, I hit the gas pedal quick, and it runs fine again for a few minutes until it does it again. It does not do this when cool.

It showed an EVAP purge flow monitor fault, and a second code of A/T torque converter clutch, no RPM drop at lockup.

The first code, I was told at the parts store, led to the canister purge valve solenoid.
I wanted to see if anyone had experienced this, and if changing this solenoid fixed the problem. Could it be as simple as the gas cap?

The second code I was wondering if this can be adjusted.

The van had a rebuilt transmission placed before I purchased it, so I suspect it needed to be set back up when put back in van.

Any help, guidance or experience is welcome!
