There are a few things you can check, the wires to the back of the switch get really hot and either come loose or the switch dies. The switches are about $10 from any parts store. The other thing is there is a resistor pack under the glove box mounted through the firewall, that sets the various speeds of the fan, and that connector may be bad. If I was to bet money, I'd pick the switch or the wiring connected directly to it. Usually if the resistor pack were to go, you would loose one speed or another only. though its connector may be loose too.
