I didn't see if it was a automatic, manual trans, or a OEM towing package.
If auto it may not be down shifting as soon as it should.
If manual user should downshift and see if it still behaves the same.
If it has a OEM tow package, use switch/button for towing. It keeps the trans in lower gear when there is a higher load.
Unless your RPM's go sky high then jurk the truck when it kicks back in, I'd rule it out.
Without details from you in different situations (towing and not towing, as well as hill no hill, hard eceleration or light, etc...). With the truck hesitating, all I can do is guess. If you could post some examples, others could give better guesses.
