If it is constantly surging no matter hot or cold - maybe the fuel pump or fuel pressure regulator on their way out? A quick pressure test would at least verify that you are getting enough fuel - may save you some headache pulling this and pulling that from the engine.

I thought the 5.9L is known for going through O2 sensors...? This can result in erratic idle and eventually a check engine light (MIL).

Have you noticed a drop in mpg?

Fuel Milage is ok got 20+ from MS to OH last month. The idling is really bad especially when cold. Will surge to over 1200 RPM back down and cycle through that several times. Will take a look at the throttle body and the valve. BTW what is the best way to clean those??

I agree with steve00ram360. I had similiar problems with my 01 Ram and replaced the Throttle Postion Sensor on the throttle body. Cost approx $30. Problem solved.

Well after finding this thread on a search, my 2001 Ram does a similar thing when first started in the morning. When I pull away from the curb, it sounds like it's missing on a couple of cylinders. If I let it sit for a few moments (a minute or less) then it doesn't do it. If I drop in gear & go, it does.

It was surging 200-400 RPM on the flat with the cruise set. I changed out the TPS (had to use a T-25 Torx driver to loosen the screws), that surging went away. I didn't notice the "miss" until after I did that. It idles fine, smooth around 1000 RPM then slows to about 800 RPM after a few moments when first started.

After the first few minutes of driving, it smooths right out. No engine codes are seen.

Any ideas? I'll try the "clean the throttle body" first.
