Default 2001 R/T Egr Issue (P0401)

I just purchased my 2001 Stratus R/T for $1300. Thought it was a steal until i started noticing things.

The first thing I noticed was the check engine light was on. So I went to autozone and had the code ran. P0401 is what came up. More specifically it was saying there was an insufficient flow. After this came up I spent tons of cash and replaced the EGR Valve, Solenoid and the vaccum lines that connect to it. Then I reset the code. Within 10 miles of resetting the code, it came back up again. Im at a loss. I have no clue what else to replace or look at. I mean the car seems to run fine except for when its at low RPMs. Idling for instance. It vibrates. The rpms dont really fluctuate so im kinda confused why it'd be vibrating.

Another separate problem thats occurring is this:
When I first go to drive the car it shifts funny. When i first start it up and drive down the road it'll go to shift and itll rev up in between shifts. This issue is intermittent. For an example: Ill be accelerating slowly from 0-45, and itll go to shift then ill lose power but itll rev up another 500 or so before actually shifting. and when it does shift, its hard. But after it warms up the shifts smoothen out and happen seldomly. any idea what this could be? The car runs fine when its in the gears so its got me REALLY confused.
