12 means that the battery was disconnected or power was not connected to the PCM for any amount of time which means that the computer needs to relearn it's air fuel mix again and that could be causing it to bog. I would check again and make sure that everything is connected just to make sure that nothing was accidentally moved out of place anyhow.
43 is cylinder misfire which could be a result of what I stated above.
I would add a bit more of seafoam too and dump the rest into the gas tank.
If the bogging persists, drive it down the interstate for 5 minutes or so. It shouldn't (*edit* sorry, didn't mean to say should, lol)stall out on you unless you got other things wrong with it, if you're worried about it. The drive will help clear out anything that maybe be causing your issue.

You might need to change your oil about 2 or 3 times yourself just to be sure all the gunk is out n stuff. I'm sure that you can just drive the car around a bit and change the oil very soon after you've cleared the smoke stage.

Like Trayton stated, you can change it right after or after a bit of driving. Just don't let it stay in there for too long because there's a possibility that something might get clogged.

As for steps, it's fairly simple, really. Suck up about 1/2 the bottle through the intake by sticking a hose down the bottle and the other end in the intake so that 1/2 gets sucked up (SLOWLY) by the vacuum of the intake, (if you want to do this) pour a 1/4, total, down each of the spark plug tubes and the last 1/4 down the gas tank and let it sit overnight. The next day, (pull the ASD relays first) and crank the car over (the spark plugs should still be removed, which means the plenum is removed also), doing this will force whatever is left in chambers to be pushed out. The rest would/should have seaped into the oil pan (this is why it's important to change the oil a couple of times and soon). You should have a big 'ol cloud behind your car after everything is reinstalled. Also note that it may be hard to start up at first, but don't worry, it will start as long as you got whatever liquid was left in the chambers out, it'll just take a good bit of cranking.
If youd don't want to put any into the chambers, skip that step and suck up about 3/4 through the intake and pour the rest into the tank. The seafoam in the tank acts as a fuel injector cleaner too. You'll see a big cloud behind your car, again, and it will continue until you've either ran out of gas or cleaned the fuel tract. I think I posted a link on how to do it either way, but I may not have. You will still have to change your oil soon because there's still a chance of the stuff seaping past the rings or just debri being scraped off and dropped from the walls.
