I have a 1994 Dakota 4x4 with a 5.9 magnum swap. After truck warms up it idles poorly and shortly after it stalls. The only way I have found to get it to pull through it is to revv the crap out of it and it then it runns fine. Also when on the road and the problem occurs it feels like it is fludding and if you pump the throttle it comes through.

I replaced the IAC and it got better for a few days

You have an EGR on that thing? I had that same issue with my '95 (5.2) and changing the EGR cured it

Alright another 5.9er. The IAC do stick alot. I had mine stick 1 week after a new one. Make sure is is free to move. clean it up and oil it up. There is a posiblity you have a vacuum leak too.
