I've removed 2 of them (from and 89 and 90 standard cab) but I can't 100 remember the process.
I removed the driveshafts, linkage, wires etc, unbolted it from the transmission, and pulled it out after moving it around. I seem to remember pulling it backwards, rotating it, then dropping it down.
It's wasn't a big enough hassle that I made a mental note.

I can't see the extended cab being any different in that section of the frame.

Maybe someone else has a better memory ?

I remember I had to unbolt the transmission mount just to get a jack under the rear of the transmission to angle/lift the transmission an inch or so, now I can't remember exactly what that cleared but that might be a trick you need to do. I think also I had to remove the large counter weight on the transfer case first.
