Hi finally had time to work on the truck again.
Disconnecting the O2 didn't make any difference.
Checked resistance on all injectors, they were all the same.
Hooked up a vacuum gauge and it was steady at 18ish
Did a compression check all cylinders were 150 - 165. All plugs were very black and smelled rich.
Swapped in a cam sensor from a running 5.2 made no difference. Still will not idle runs like crap and no codes.

This has us stumped.
Were rechecked the fire order, vacuum lines they are correct. Fuel pressure is within specs and it holds pressure, Computer still has no codes. EGR checked, changed coil, checked the timing it's with specs, checked the distributor it is synched within specs.
It still runs like crap, will not idle, backfires through exhaust and will only keep running if over 2k rpm. After a while of running at 2K the drivers side exhaust manifold gets glowing orange the passenger side does not.
The data on the scanner says it is running lean, but the exhaust smells and looks rich.

Engine has to be at temp, and then the scanner will set the rpms to 1000 to lessen valve train fluctuation.
Does this truck still have a cat on it?

It was an old Snap On scanner that had provisions and a plug for OBD1. The fuel sync was +-3 i don't remember which, we figured that was close enough I had seen online that up to +-7 was ok.
It will only run at 1k for at most 10 seconds then it cuts out. The scanner data showed a lean condition then kept adding fuel till it cut out. Exhaust smelled rich and all plugs were very black.
If we keep it at 2K it will run but shoot flames from exhaust and drivers side exhaust manifold is glowing orange.

IT holds pressure. So far everything we check is normal or within specs. It’s frustrating!
