From the factory service manual, is one source.
I also use http://www.dakota-truck.net/CODES/codes.html when I'm feeling too lazy to hunt up the proper page in my FSM
Just be aware that some codes may return multiple possible reasons; use some thinking (i.e., code 45 for instance won't be the OD solenoid if your truck doesn't have overdrive ... )
