Default Small amount of oil in coolant

There is a small amount of oil floating at the top of my coolant reservoir. Does this automatically mean my head gasket’s going, or could it be the oil cooler going? The oil cooler does look a bit rusty.

There really isn't a way for oil to get into the coolant via the engine. Pressure testing the cooling system wouldn't hurt though. Flushing the cooling system is probably overdue.
