any CEL codes? if it were something in the driveline (wheel hubs brakes driveshaft out of balance i would expect it to be continuous not intermiten) almost makes me think the engine is missing bad (ONLY when it is shaking)

btw it sounds to me like you feel the whole truck shaking, not just the steering wheel,

also what engine do you have? the 4.7 or the 5.9? (yes it makes a difference in the trans/driveline that was used)

this is a long shot, but check your balljoints, jack up the front and pull on the wheels at 12/6, if it moves replace the balljoints,

and is it always shaking when you are driving or intermittenly? (trying to narrow it down for ya)

Does it happen when you hit a bump or rounding a corner? When was the last time you replaced your shocks?

I let my shocks go over 100K then asked why my truck would shimmy when I was rounding a corner and hit a bump, sure enough it quit when I replaced my shocks. Not only that it rode like a new vehicle! If it shimmy's while going straight on a smooth surface then most likely you have a bad ball joint or something.
