Well my friends 1999 5.2L D had the fan come flying off! It took out the water pump, radiator, and fan assembly. $780 for all out at the shop. Man with 80K I'm surprised it happened. His has the pulley that is pressed on the water pump and it gave way.

 

Ouch.
