Default bottom end rpm good, high end rpm weak

in the low rpm the engine pulls like crazy, but after about 5k it feels like there's nothing there. If i back out of the throttle a tad it seems to pick up a little better past 5k. Wondering whats the deal... I have a header, high flow cat, diy cai, and 2 baffle exhuast (basically the closest you can get to a straight pipe exhuast legally). i think it's air/fuel mixure issues, or i should spring for a set of cams. Also i proudly have 275k on the engine and transmission, also i think the differential may be ready to fly apart... I have work to do aparently but i dont know where to start.
Thanks
Garrett

When was the last time you did a tuneup? Spark plugs, wires, PCV valve and so on...

oil change is coming up soon. Plug wires are about a year old (30000-50000km) they are accel plug wires. Standard plug wires (gapped right and all) those are about 10000kmish, and the pcv valve hasnt been done, and i don't really know what that is?

Positive crankcase ventilation valve. Its about a $5 part. It would be attached to the hose that runs from the top of the valve cover down air intake. If it is bad or clogged, it can cause wired issues.
