Default 93 d150 jumps bad when switching to a higher gear

Ok I replaced the 1st piston, rebuilt the head, replaced and checked vacuum lines, replaced iac valve, cleaned and swapped some injectors. I finally got the truck to idle and run but now when I switch gears it gets all jumpy, jerking around, I'm not sure of what to do, any suggestions would greatly be appreciated.

Okay to add to that it feels rough when idling.
