Problems while driving

I have an 89 W150 with a 318 manual transmission. The engine starts and idles very well. Once in gear and driving the engine seems to flood out as RPMs increase. Doesn't seem to be a problem if I am extremely gentle with the accelerator. Normal driving is problematic.
