Default code 1391

i got a 99 dakota and in the last few months i got a code 1391(intermitten cam or hall effect sensor or crankshaft sensor).i replaced them both along with plugs wires and rotor.the truck works good for a while and then it will miss and idle erratic while in gear,put it in nuetral and you can rev it up like nothing is wrong.then slip it in gear maybe it will go for a week or 20 feetbefore doing it again.im getting that code again,surely it cant be gone again, any ideas?

P1391 Intermittent loss of CMP (camshaft position) or CKP (crankshaft position) signal
Camshaft Position (CMP) sensor and Crankshaft (CKP) sensor signals are monitored with engine running or cranking. Diagnostic Trouble Codes (DTCs) will set in Powertrain Control Module (PCM) memory when the PCM senses failure counter reaches 20 failures for 2 consecutive trips.
Possible causes are:

* CKP sensor improperly installed.
* Defective CKP sensor.
* Defective CMP
* Defective PCM
* Defective tone wheel/flexplate.
* Visually inspect related wiring harness. Check for chafed, pierced, pinched or partially broken wires, defective connectors or defective wiring.
* Visually inspect related wiring harness connectors. Check for broken, bent, pushed out or corroded terminals.
