Default Manual Tranny problems

I have a 2000 Dakota 4.7L 4WD that has been slipping when i got over 3000rpms or so. I thought i was looking at needing a new clutch but have some other things that have occurred the last few days. I have noticed a loud humming which i thought might be the rear end, checked the rear end lube, which was full. When i got in the truck the other day and put it into reverse i had nothing but had all my forward gears. Later on i found that when i put it into reverse, it goes into forward, not sure what gear tho. I am totally lost on what could of occurred. This is a manual trans also i should mention. Any help on this would be greatly appreciated, Thanks!
