Default Oxygen sensor help please

Ok so I have been reading all these posts and threads in this and other dodge forums and still haven't found anything that helps me in my current situation.
My mother has a 2001 Dodge Dakota SLT w/4.7. The PCM kept overheating. It would get hot and you could not start truck again until it cooled down. I bought a Mopar replacement PCM installed and had it flashed. Truck ran great. Got a miss fire code about 6 months later. Replaced plugs, coil packs, injectors TPS and nothing worked. Finally pulled bank 2 head and found #8 valve spring had snapped in two. Pulled both heads and them rebuilt. reinstalled everything. Truck runs great now...Except.
No I get multiple codes that I can not get to clear in order to get it inspected. I get P0138 and P0141. I have replaced Bank 1 Sensor 2 3 times thinking maybe I bought bad sensor. Replaced Bank 1 Sensor 1 and Cat. Here is where it gets weird. I get both P0138 and P0141 codes. when I clear them with my OBDII I get no codes as MIL reset then I will get 3 codes (P0141, P0442 and P0455) I know the 400 codes are small and large Evap leak codes. Clear codes and when MIL resets I get the original 2 codes. Back and forth. I have replaced vacuum hoses and tested for vacuum leaks but come up with nothing.
Not Sure why the codes keep changing from one reset to the next. Completely confused on issue and have replaced close to $1000 worth of stuff so far. Any ideas on what I might be over looking?
Thanks.

What kind of O2 sensors are you using? DON'T use Bosch sensors. They don't work well in Dodge trucks. Just this past Sunday I replaced all my Bosch O2 sensors with Denso brand which is OEM. That fixed a problem I've been having for the past 2 1/2 years. I'm not saying that this is the problem you're having, but if you do replace the sensors, I recommend Denso brand. I believe different brands have different reaction times which causes problems. My Dakota has the 3.9L Magnum V6 which uses four O2 sensors. If you do decide to replace the sensors, check out Rockauto...they have pretty decent prices in comparison to the auto parts stores.
