2000 4.7 strange rattling noise at 2500rpm

2000 4.7 strange rattling noise at 2500rpm

I just picked up this 4.7 4x4 with 127k miles it runs and drives great, the only thing is I hear this weird rattling noise only at 2500 rpm super low speeds, I can’t hear it while driving only if I’m in neutral or accelerating slowly from a stop, sounds like it’s coming from the drivers side head, there is also an obvious exhaust leak on the drivers side headers as well so maybe that is why I can hear this noise? Any advice on what the noise could be would be super appreciated
