Default Serpentine Belt Slipping off

Twice now within the last month the serpentine belt on my 1998 Dodge Dakato V6 slipped off. Anyone ever had this happen and what would be the cause? The first time it happen I changed the belt, but now it happen again.

I'm sure the tension on the belt is tight. I've noticed though that the belt wants to slipped toward the engine and then it would stopped. Maybe the tensioner pulley needs replacing? Also, I wasn't driving it when this last time it happen and I'm concerned how far it went without the belt. I'm at work now and the truck is at home so I'm not sure where the water pump is. If the belt drives the water pump then more than likely the engine has been damaged.[:@]

It's fixed. It was the tensioner pulley. Also, the belt must have slipped off when it was last started. No problems with overheating, etc.
