I am looking to see if the following truck was available from the factory and all of my searches have come up with nothing...
- 1998-2003
- Ram 2500 or 3500
- 5.9 Magnum GAS / 360ci
- NV4500 manual transmission

Bingo^^^ It all depends on trim and model. For instance, my ST came standard with the 5.2 and NV3500 with an optional 46RE, and an optional 5.9. An SLT came standard with the 5.9 (auto) but could be "downgraded" to a stick and 5.2. The NV3500 was only rated for 300 lb/ft whereas the 5.9 offered 330 lb/ft which is why the 5.9 was never offered with a stick in the 1500 frame.
Since the 5.9 was the baby engine for the heavy duties (98-03), it was standard with stick as well (NV4500) but for a different reason. Back then, autos were actually weaker than sticks for heavy duties and were more of a convenience/luxury option (all three manufacturers offered sticks). All three engines in the 2500 and 3500 came standard with sticks (NV4500, NV4500HD, NV5600) but were available with autos as a trim upgrade except the 5.9 Cummins H.O. (2000-01) which only was mated to the NV5600. Light Duty 2500s were standard with the 5.2 and NV3500 until discontinued in 98.
Sadly, or rather tragically, as of 2015, the only new trucks you can order with a manual transmission are Chevy Colorado (2wd), Nissan Frontier, Toyota Tacoma, and the Ram 2500-3500 with the "low output" Cummins 610 lb/ft (G56). So autos have become better, but America got wimpier. There is no V8 gas engine mated with a manual in any new showroom in America.

Well not for trucks. Can still get a challenger with the manual
It's less that standards are inherently weaker, just that the public doesn't care for sticks at all anymore so the manufacturers don't design them. It's amazing the Cummins still has an option. I've actually seen a few brand new ones on craigslisy witht the stick.
I think it's just a G56. So it's not even new or special.
