Sounds like it could be one of several things wrong. The sucking sound could be an intake leak that is causing the PCM to adjust and readjust the idle. Could also be a bad TPS or IAC. Any codes?

does it run ok other than idle ?

if so, have you replaced IAC. i'd try that first.
if that doesn't fix it, i'd do TPS next.
you generally cannot return these parts. so keep your old one's, and if the new one's don't do any different, then keep the old as spares (for the next time).

edit - even if it is an intake leak, its the IAC's job to adjust pintle in/out to maintain a desired idle speed.
last winter mine took a "surging fit" sort of like that. it was the tps.

+1 for vaughan's suggestions. IAC then TPS.

Did you use a gasket between the air cleaner/hat and the TB?

I thought I suggested those in my first post.

That might explain the hissing sound, but not the poor performance.

So you did, so you did. My apologies.

+2 then.

That might explain the hissing sound, but not the poor performance.
It won't explain everything, but it might be adding to a bad IAC or TPS or other problems.

ok get some carb cleaner and start the truck up and start spraying the engine down where all your vac lines are when it revs up you just located the air leak. Also have you done a fuel pressure check on it ?

before you go buy a new tps or iac test them to make sure they're bad, it'll save you 80$ if they aren't. tps with a multimeter and iac with a couple different tests. grab a haynes manual and it has the test procedures in it. even if the iac turns out to be good it wouldn't hurt to clean it and the hole that it plugs into.

i believe the map sensor could have something to do with it, not sure but worth checking out. haynes tells how to test it also.

even with a gasket between the air cleaner and the tb it may not be sealed all the way around and that'd explain the hissing noise. i know when i start mine with the round air cleaner it has a really loud hiss from the tb. i couldn't hear it with the stock box, then it was noticable with the cai and now its extremely loud with the 14x3, but only for about 30 seconds.

after you test the fuel pressure, tps and iac tell us if its still running rough.
