Question Engine or fuel pump problem?

Ok, I need help. I have a '00 5.9L 1500 with about 170,000 miles. I was driving home when the truck started sputtering and would die at stop lights if I didn't keep the RPM's up. It idles very rough and surges. If I rev the engine in neutral, it acts like it will or actually will die. I can start it right back up though. I looked through the Haynes manual and it said it could possibly be a vaccum leak or a fuel problem. There is a very loud "whirring" (good description huh?) coming from the fuel tank. I know that the pump will make this sound when first powered up, but the manual says it should go away when system pressure is brought up.

Has anyone ever experienced this before, and what did you do to fix it? Any other ideas what could be wrong?

Thanks for the help!

if you can keep the truck running by holding light throttle pressure, its probably not fuel related. could be a very bad vac leak, bad IAC or TPS (common), or a bad skip with two or more cylinders missing.
