Default Illegal Intake, Trying to smog, have stock parts but ?? (+pics)

Please excuse my lack of knowledge. Been riding motorcycles most of my driving career and this is my first vehicle. So I took my truck in to a test only center to try to pass a smog test. The technician said my intake setup is illegal and he can't legally run a test. This is how it looks right now:

When I bought the truck the guy gave me this:

What other hardware (bolts, clamps, hoses, ect.) do I need inorder for me to go back to stock so I can be "legal"

Nothing. Just bolt on the stock air box. I have a hunch you live in Cali.

there should be a filter for the box and a hose clamp to attach the box to your intake. the tube goes from the box to the fenderwell

Put on the factory airbox, plug the crank case hose into the side of the airbox, and go get smogged. You are failing because that round air cleaner doesn't have a CARB Exemption number on it.

funny I live in California and have that same filter just a little taller filter, Have smogged it two times with no problem.

I am guessing there problem is they can not see were the crank case vent goes.
To be legal all crank case gases must go back into the intake.

Just my .02

No, the problem is that they can't see the CARB exemption number. It is a checkpoint on every smog test done in this state. If you've been passing, its because the place you're taking it just dgaf. Keep going there. The TB spacer isn't causing the failure. It more than likely has a stamp on it.
