Default Fuel system problems?? Need some help.

Alright i have a 1997 1500 4x4 with a 5.2v8 and 190,200 miles. The motor is all orginal and i dont know what stuff has been replaced on it. Anyways, if i drive it to the store or something and come out like 25min to 2 1/2 hours later start it and take off i will give it gas and the RPMs will go up, then start going down and the truck starts popping and jumping. It acts like its not getting any fuel or something. Then i can let off the gas and get back on the gas and it runs perfect the rest of the time. Also, when it is started cold and i first start taking off it does the same thing except not as bad. It just feels like its doing everything it can to get going. Not much power at all. Once it warms up a little bit though, it runs perfect. Lots of power. I know i have a vaccum leak somewhere because the truck doesnt idle right and stalls sometimes. Also i have wild vents. Trying to get all of this stuff fixed. I dont think this vaccum leak is causing me the problems i stated above though. I think it's fuel system related. Anybody have any ideas? This problem is really stressing me out.

Thanks.. Any help??????????
