Cold Idle issue - truck whines/hisses and RPMs drop way down almost to stall.

Yes there is a filter in the breather. It should be on the drivers side valve cover, usually a brass colour. Yes it should be replaced on a regular basis. I recommend checking it with every oil change by removing it and shaking it upside down. If a lot of stuff comes out or you hear rattling inside, replace it. Remember to put about a tablespoon of enging oil in it before putting it on the engine.

Some have the 'breather' type feller, others just have a plastic elbow on the 'intake' side of the PCV system. Varies by year. Later years just had the elbow. I don't think that the larger version actually filters though, as it is already drawing air on the 'filtered' side of the air filter.

Yeah, my 96 has the big steel can thing on the drivers side valve cover, but, there is nothing in it, aside from a baffle..... no filter at all.

That baffle is the filter, or so it is called. How good of a job it does I don't know.

I don't consider a flat piece of steel to be much of a filter. More of just a simple restriction in the air stream to control flow should the PCV itself fail I suppose. The air at that point should already be filtered, not really much point in filtering it again.

Hmmm... The ones I have have a fiber disc of some sorts inside of them... what brand are you using?
