It is almost like the truck is detuning itself or something.
My first thought was fuel pump or bad gas.

Can't be gas since it has been doing this for a long time, many, many tanks. I won't rule out fuel pump cuz I have no idea what the problem is. I have had fuel pumps and filter clog before and they always rand for a while and died. This runs like a top, just no power.

Are you sure that's not the PCV? There is a crank case breather on the passenger side that goes to the air filter.

Should have a little elbow looking like thing on the valve cover towards the firewall.

Here's what mine looks like:

I have the PVC hooked to the stock air cleaner. There is a little nipple for it.
This isn't mine but looks like this.

i have 2 ports on the driver side of the manifold of my 01. 1 goes to the pcv, the other to the brake booster. Passenger side has a port that goes to vacuum lines for the 4x4, vents, cruise and whatever else is spliced into it.

My crankcase breather on the pass side valve cover goes straight up to the airbox. Not sure if that helps but that's how mine was setup. From the pic u have, that open port would be the one that goes to the pcv valve on the driver side valve cover. At least that's how its set up on my truck. And yeah...that'd be a pretty major vacuum leak if its wide open like that.

I just plugged it, I will take it for a drive tomorrow over my commute hill and I can tell if it made a difference. Just drove around the block and it seemed a little better. But need to get in to drive over that hill and see if she pulls better.
