There's a shift collar for hi/lo range inside the case. Under that is low range gears on the countershaft. Low range consists of gears set of four. Biggest gear mounts to a intermediate shaft, which is not pictured, and is almost a size of that hole. In order to get 4:1 gears it would have to be much, much bigger. And as it can be seen, just impossible to fit in there.
Maybe it's a custom case like Heik said I'll message the guy and see what he said

I tthought the 203 was a full-time 4wd, chain driven case. The 205 was gear drive???

So the guy got back to me and said he doesn't know it was his dad's. I thought the case looked to new, but again I did verify it has 4.10 by engaging low and spinning the input shaft.
