That's funny I didn't watch the vid but did the same thing with a paint pen on our '02 2500. The weird thing with rear discs and DIH parking brake is that it doesn't necessarily seem to matter which way you install the adjuster. It's not self-adjusting so there's no clicker arm, just a spring that kinda keeps the adjuster wheel from turning on its own

I recently replaced the shoes and tried the adjuster both ways -- it didn't really seem to make a difference-- but none of this helps the OP if he's working on dedicated drums

You'll see lever behind the star wheel, and if everything is in good working order and nothing is tight or binding, that lever should turn the star wheel down to automatically adjust the brakes. If you need to loosen the shoes, you'll have to tap that lever after every small turn of the star wheel, so the wheel notch gets past the lever.

The star wheel on the adjuster will only really go one way. It catches, if everything is in good shape. Before you go to that much trouble, why not just adjust it the way it was intended. Since the truck is rarely driven, it may not be braked while going in reverse much. Get out on a rural road with little or no traffic, drive in reverse and jam on the brakes. That's how they are designed to be "self adjusting".8 or 10 good stops and I'll bet they tighten up nicely. You may hear some popping or clunks from the back end.
