Default 97 Ram overheating at idle

I've got a 97 dodge ram 5.2. It has a 195 degree thermostat but runs at about 220 when driving. If I'm stopped for 5 minutes or so the truck will get all the way up to 250 right before the recline but as soon as i start driving it'll go back down to 220 ish. Is it not getting enough airflow from the fan or what?

Sounds like a bad fan clutch.

When you first start it rev the motor up and keep it there, You should hear the fan roar.

Im not sure it's the fan. When I'm stop after driving the temp gauge will go up like it's trying to overheat and the air starts blowing cold. Would that be a clog or air trapped in the system?

As said above when first starting up and then a little throttle you should hear the fan roaring, if not it is the fan clutch. Now if you have stopped for 5 minutes and the temp goes up then out on the road going at least 30-45 mph you are pushing air through the radiator thus providing enough air to cool it down, check the coolant level also. Been there several times with a bad fan clutch on different vehicles.
