when you cleaned the TB did you make sure the air passage is clear? IIRC there is a small (real small, about the size of the extender tube on spray cans) air hole that could be partially plugged.

did you do timwags suggestion? generally your pressure would go down not up but it never hurts to check.

I'm having the same problem with my 2000 RAM, only i have the 5.9L and it will do it about everytime i start it if it has time to cool a little. I have cleaned the IAC also. I also hear the load vaccum sound for a few seconds. Hoping to hear what you find out anything. I'll let you know if i find solution.
