Take the TB off and clean it. Especially the IAC hole. The IAC itself gets gunked up alot but theres usually even more in the hole on the TB. Mind had all kinds of crap and gunk in there.

No paper gasket but there should be a rubber o-ring. My new one didn't come with one and but I re-used the old one.

The O-rings should always be replaced with a new one. When replacing the O-ring, it should be lightly coats with oil so as not to be torn during assembly.

Could be, I noticed today that the TB looked pretty dirty, I didn't remove it from the engine but I did spray some carb. cleaner in there while I had the IAC off.

not really too good to spray carb cleaner in tb while its still on the engine because all that crap falls into the intake manifold.
