I'd guess a bad synchro; remember, modern transmissions don't shift gears, every gear is always in mesh. You just shift the dog clutches that engage the gears to their shafts. Hopefully it was just a bad part; unless you were going out everyday and powershifting or performing clutchless upshifts.
