If you do an internet search on headache racks, you'll find a lot of ideas to make a custom build. If you don't have your own welder or aren't good at it, you might find it simpler to buy one already made. If you choose to fabricate your own, you'll need to make mounting provisions on the front rail and floor of the truck bed, the rail mount provisions will hold the rack tight to the truck and the floor mount will keep it from folding forward or backward under load, also, ensure your mounting points are reinforced so that the bolts don't pull through under load.

To make measurements, I would get a piece of lumber (a straight piece of 1 x 4 would be nice) and lay it on the front rail. Mark the length you want and cut it to size and then clamp this onto the front bed rail of the truck. Now with some cardboard and duct tape, you can make the other parts of the rack that you want & when you're done, you'll have a template and you'll be able to use that to determine what steel you need & how much. You'll be able to drill all your holes in the wood for your mounts and if you screw those holes up, it doesn't matter, it's just your template, but in the end, you'll have the plans for a finished product that will fit and look the way you want. It's a lot easier using scissors and tape than a welder and angle grinder.

If you end up welding on the truck, don't forget to disconnect the battery so you don't fry the computer.

I have one I built for my truck. I can get the measurements if you'd like. I can maybe send a pic too.
