If it were me I'd wait and see how the truck does w/ the new tires and wheels and then decide if you wanna drop more cash on regearing. If your mpg stays about the same and the you don't have to downshift anymore than usual I'd leave it alone. I went through a couple bad gear installs on my truck and it really wasn't worth the headache or the money.

W/ a manual tranny it's just gonna shorten how long you're in each gear, like you'll have to shift out of first at a lower mph than w/ 3.55's. If you want some more power a tuner from hemifever isn't a bad idea, a lot cheaper than gears and can easily be removed.

Your way under average with your '03 UNLESS it's the bastard truck (5.9 in an '03 1500) in which case you are right around average. If you are only getting 9 mpg with a 4.7 or Hemi and it isn't lifted with big meats, you have a problem.
The '04 is getting better gas mileage than most, I know three people with RCSB 4x2 V6 1500s and they don't get any better gas mileage than guys with 4.7 4x2 QC automatics.
I don't have an issue with the V6 in a RC 4x2, it's not that heavy of a truck.
But, with a manual transmission, re-gearing isn't going to give you much of a mechanical advantage, you'll actually achieve a much greater advantage just simply dropping down a gear or two with the shifter. At wide open highway traveling where you are just cruising along in 5th, I'd bet good money you'd actually see a DECREASE in fuel economy with numerically higher gears.
Yeah, 22s are gonna effect fuel economy negatively, if you want 22's just accept it, just like all of us guys who lift our trucks do...
