back the fuel nozzle out of the receiver a bit when filling. For some reason, when you bury the nozzle it won't stay running.

This girl I was seeing before I got married had the problem with her car. The fill tube had gotten bent. She put 2 +2 together and figured it started happening after she ran out of gas once and her ex bf came with a can of gas (now I got married in '93, so this had to be around '89 or '90 back in the days when gas cans & spouts were metal). Had to actually take the side out of the trunk to get to it. You should be able to see the thing from underneath the truck. Worth a look anyway...

That's what I'd put my money on, a bent fill pipe. It causes turbulence that tricks the nozzle into sensing that the tank is full.

the metal flap that would be right on the opening of the spout right or farther down the neck

From your pic I don't see any damage. You should be able to see the flap right there when you take the cap off. It gets pushed open when you put the nozzle in. If it's not there maybe it was knocked off and is stuck down in the pipe causeing a restrition. If it is there maybe something else was put into the pipe and it is causeing a restriction to flow.
