What Mods Are A Must??

The mods to look at right away would probably be the big three of CAI, exhaust and tuner. Personally, I like the tuner first bc I feel like you get the most bang for the buck from it but others like to do the supporting mods of CAI and exhaust first, just personal preference.
CAI's are all about the same, except for the BFI. The BFI seems to get a little better performance than the rest but costs a lot more.
Exhaust, you just have to decide what sound you like and what kind of flow you want. Flowmaster is a sound that a lot of people like but they don't flow well (and rust out in no time). Magnaflow flows better than FM but doesn't have the same sound. Then there are the AT3030's and so many others, just check the FAQ and you will see all kinds of muffler info. Personally, I like the e-cutout, best exhaust set up I have ever had.
Tuners, well you didn't give any info as to the year of your truck or the engine you have so I can't really give you any advice as to what tuners would or wouldn't work for you.
You mentioned the e-fan, that is a mod that I have and it is a very good one. I put a fan from a 94 Ford Taurus in my truck and it cools like a champ. It also is the best MPG improvement that I have gotten from any mod.
There are a lot of other mods you can do, but I think the big three should give you a good start.
