Most of us fantisize about great mileage as we reset the computer at 75 miles per hour and coast to the red light.
My 2003 Quad Hemi with tow and 4x4 (3.92 dif) has over the past two years and 49,000 miles averaged 14.9 MPG including some 4x4 and towing. (That's actual calculated MPG over the entire life of my truck - not the 16 or 17 you may get on stretches of road.) I drive conservatively with some outbursts of unrestrained heavy pedal driving.
The best way to increase mileage is to drive as if an egg were under the pedal and use cruise on open roads whenever possible. Otherwise realize it's a truck and have some fun.

ive yet to break the 15mpg barrier and i do alot of highway driving with some intown too. its hard to not have a heavy foot with this manual but even when i did take it easy i couldnt break 15. just have to accept that its not gonna get much better, we drive full size trucks

As to the original question, I don't think your friend will find a magic pill to his problem. He may spend several hundred dollars to get maybe 1 mpg difference, but probably won't make up the cost. If most of his driving is stop light to stop light driving, he probably won't get much better. His driving habits probably play the biggest role. My 4.7 Dakota only averaged about 15. My hemi consistantly get about 1 mpg better.
I don't really know what I would get continuous city driving. I would guess 13 or worse in continuous stop light to stop light driving. Since I live in a rural town and work in St Louis, most of my driving is about 50% city & 50% highway most tanks consistantly calculate between 15 and 16. My 4.7 dakota probably got better MPG in the city, but the ram gets better on the highway.
