Why would there be a high amount of vacuum in the crankcase? Also, this car runs fine as long as the oil filler cap is removed; but bad with it on; plus it smokes.

Because there is a breach between one of the cylinders and the crankcase. Or your PCV is stuck.

sounds like its not venting properly, pcv valve might be stuck close or the breather/vent might be clogged
